Pr\u00e9sentes dans chaque service de soins, elles vous informent de vos droits, vous aident dans vos d\u00e9marches administratives et vous orientent en fonction de vos besoins.<\/p>\n<p>Elles peuvent intervenir \u00e0 tout moment dans votre parcours de soins \u00e0 la demande :<\/p>\n<ul>\n<li>de vous-m\u00eame ou de votre entourage<\/li>\n<li>de l\u2019\u00e9quipe pluridisciplinaire<\/li>\n<li>des services sociaux municipaux et d\u00e9partementaux<\/li>\n<li>des partenaires ext\u00e9rieurs<\/li>\n<\/ul>\n<p>Les assistantes sociales travaillent avec vous \u00e0 la mise en place d\u2019un projet individualis\u00e9 en collaboration avec les \u00e9quipes pluridisciplinaires.
